OrthoArizona offers a wide variety of services that include physical therapy, diagnostic testing, casting, durable medical equipment, and workers’ compensation.

OrthoArizona offers Medical, Dental and Vision plans for both you and your family members. We also offer Virtual Care that provides you and your eligible family members access to medical providers via the app anytime, day or night, and from anywhere with an internet connection/Wi-Fi.
Health Savings Account
When you are enrolled in the UMR High Deductible Health Plan (HDHP) and meet the eligibility requirements, the IRS allows you to open and contribute to an HSA Account. OrthoArizona contributes $25 to your active HSA per pay period.
Basic Life Insurance
We understand the value you bring to both the company and your family. OrthoArizona pays the cost for Staff to have $25,000 of MetLife Life and Accidental Death and Dismemberment (AD&D) Insurance.

Voluntary Short & Long Term Disability
One of the most important assets to you as an employee is the ability to earn an income. OAZ offers voluntary short term (Employee-paid) and voluntary (Employee-paid) long-term income protection through MetLife in the event you become unable to work due to a non-work-related illness or injury.
401K Plan
For full-time employees: If you have worked a continuous six months and are over the age of 18, you are eligible to participate in the OrthoArizona 401K Retirement Plan.
For part-time employees (working less than 29 hours per week): Once you reach age 18 and have completed 1,000 hours of service and/or one year of employment, you will be eligible to participate in the OrthoArizona 401K Retirement Plan.
